ASSESSMENT APPLICATION INSTRUCTIONS
I. Prior to starting your grant application we suggest you gather, scan, and/or complete all of the following:
PLEASE UPLOAD ALL ATTACHMENTS in one of the following formats: EXCEL, CSV, WORD, PDF, TEXT, or RICH TEXT FILE
- The GtG Budget Template (Word document - please fill out and save for uploading later using link in application)
- Copy of Internal Revenue Service's current 501 C (3) designation letter or a copy of your IRS 501 C(3) number (please have it saved as "A. IRS 501c3 Designation" and ready to upload using link in application)
- Current Business or Strategic Plan – entire plan (please have it saved as "B. Business Plan" or "B. Strategic Plan" and ready to upload using link in application)
- Scanned copy of signed and dated deed proving building ownership or lease agreement proving long-term lease for at least 5 years or more (please have it saved as "C. Deed" or "C. Lease Agreement" and ready to upload using link in application)
- Scanned copy of a letter from Board of Directors signed by the organization's Board Chair that includes a statement showing the boards commitment to participating in the Grants to Green program. (please have it saved as "D. Board Letter" and ready to upload using link in application)

List of Board Members, including occupation, city/town of residence. (please have it saved as "E. Board Members" and ready to upload using link in application)
- Assessment Match Commitment Form (please complete and submit electronically)
- Green Champion Information Form (please complete and submit electronically)
II. Complete and submit this Energy Assessment Grant Application
